Code of Conduct Report Form
When reporting you must report enough facts (who, what, when, where, how) to allow the Company to follow up and investigate the report. Reports of suspected wrongdoing should be factual rather than speculative or conclusory and should include all supporting or corroborating information known to the individual making the report. Reports must include sufficient factual and corroborating information to support the commencement of an investigation.
